✅ Step 1: Complete and Upload These Forms
Before your interview, please download, complete, and upload the following forms to your YNA Portal Profile.
Then bring the original signed copies with you to your interview.

💡 Tip: Uploading these before your interview helps speed up onboarding and avoids delays.
🕐 Step 2: Upload Additional Documents to Save Time
If available, upload the following to your YNA Portal and bring the originals for verification.
🪪 Identification – 100 Points of ID
Include enough documents to total 100 points:
🛂 Current Passport (and Visa if applicable)
🧾 Australian Birth Certificate or Citizenship Certificate
🚗 Full Driver’s Licence
🧾 Two other forms of ID (e.g. Medicare Card, Bank Card, Utility Bill)
👩⚕️ Professional & Training Documents
🧾 AHPRA Certificate or Card (current)
🎓 Degree, Diploma & Transcript (original)
🧠 Post Graduate Certificates (if applicable)
🧸 Working with Children Check (current)
🩺 CPR Practical Training Certificate (completed within 12 months)
💪 Manual Handling Training Update (completed within 12 months)
🔒 Screening & Compliance Checks
🕵️♀️ Police Clearance (CRC – National or Federal) issued within 12 months
If your CRC is not current, YNA can assist with an application at no cost
🧾 Recent Payslips or Statement of Service
🪪 Current DCSI or NDIS Screening Checks (e.g. Aged Care, Disability)

Before your interview, make sure you’ve:
✅ Uploaded all required forms to the YNA Portal
✅ Brought all original documents for verification
✅ Completed all mandatory declarations and training certificates
✅ Confirmed your Police Clearance and ID are current
